CHIEF COMPLAINT: A 14 yr old male patient came to causality with a complaint of stomach pain since 15 days
HISTORY OF PRESENT ILLNESS : A 14 yr old male patient who is student by occupation was apparently asymptomatic 15 days back then he noticed continous dragging type of pain in the abdomen.
First patient visited local hospital and then came to causality

History of similar complaints 5 months ago
H/o chicken pox 1year back
No h/o hypertension diabetes epilepsy asthma thyroid renal and cardiac problems
FAMILY HISTORY :
Similar complaints of his sister who passed away

No addictions
No known history of drug allergy
GENERAL EXAMINATION: patient is conscious coherent cooperative and well oriented to time place and person

CVS: no thrills and cardiac murmurs

No neck stiffness
